  • "Long Covid is a distinct condition affecting approximately 60,000 people in the UK ... Clinical guidance and local protocols for long Covid are currently highly variable. This is likely to be contributing to the patient experience of inconsistent and fragmented care."
  • The paper recommends that a 4-tier clinical service be developed: a) Tier 1: resources and support for self-care. b) Tier 2: generalist care including a therapeutic relationship in general practice and a community-based interdisciplinary rehabilitation service led by allied health professionals. c) Tier 3: specialist care including system-based investigation, management and rehabilitation. d) Tier 4: specialist management of specific complications.

  • An evidence review on ‘multiple site, single service models of care’ (MSSS) has found evidence of 10 key enablers and barriers to implementation of MSSS models which will help NHS leaders better plan, articulate and deliver their MSSS models of care. 
  • The review also identified shared characteristics which enabled the development of a descriptive Clinical Framework that for the first time can be used to support NHS systems develop their MSSS models of care.

  • ‘Multiple site single service’ (MSSS) models of care describe a method of delivering care across more than one clinical site or location, often across a regional geography, through a single clinical service or team. This systematic review examines the published evidence around MSSS models.
  • It also sets out a universal framework to categorise different MSSS models which may be employed in the reorganisation of secondary care services, and a suggested set of outcome measures to guide planning, implementation and evaluation of future clinical service reconfigurations to better understand the population impact of service change.

This paper reports the results from the first year of the St George's Hospital hyperacute neurology service (HANS). The outcomes include admission avoidance from ED, and a reduction in length of stay for non stroke patients and reduction in occupancy of stroke beds by non-stroke cases.

  • The review outlines the challenges and benefits of working in general practice and of the Partnership Model. These include workforce issues of recruitment, retention and education and training; workload, status and role of partnerships and primary care networks; and the emerging opportunities for primary care in terms of digital and technology.
  • The review concludes that "the current model of care in the NHS is too dependent on hospital-based care. This model is not sustainable, and we cannot move forward without change that includes general practice and partnerships at its heart."
  • A separate report presents case studies of different business models in general practice.

  • This guide sets out a vision for strengthening general practice collaboration across London.
  • It outlines two main types of collaborative arrangements between practices:
    • Larger scale general practice models (LGPOs). Multiple practices work via formal collaborative arrangements so they can develop and train a broad workforce, as well as create shared operational systems and quality improvement approaches.
    • Primary care networks (PCNs). Practices come together with community providers and the voluntary sector to serve smaller populations. The vision is to create multidisciplinary and team-based care for people with enduring, complex health and care needs.
