Criteria for evaluating transferability of health interventions: A systematic review and thematic synthesis.

  • This systematic review aims to develop model for the assessment of transferability of public health interventions through identification and systematization of influencing criteria, including facilitators and barriers. 
  • Synthesis resulted in two related models: the conceptual PIET-T model explains the underlying mechanism of transferability of health interventions and the PIET-T process model provides practical guidance for a transferability assessment.

Kirkup report highlights failure of lead commissioner arrangements

  • The Morecambe Bay Investigation looked at the maternity and neonatal services in University Hospitals Morecambe Bay NHS Foundation Trust.
  • Covering January 2004 to June 2013, Kirkup Report concludes the maternity unit at Furness General Hospital (FGH) was dysfunctional with serious problems in 5 main areas:
    • Clinical competence of a proportion of staff fell significantly below the standard for a safe, effective service.
    • Poor working relationships between midwives, obstetricians and paediatricians. 
    • Midwifery care became strongly influenced by a small number of dominant midwives whose ‘over-zealous’ pursuit of natural childbirth ‘at any cost’ led at times to unsafe care.
    • Failures of risk assessment and care planning resulted in inappropriate and unsafe care.
    • Grossly deficient response from unit clinicians to serious incidents with repeated failure to investigate properly and learn lessons.
  • CCGs had not inherited a service specification from the PCTs and the absence of lead commissioning arrangements inherited from PCTs was found to have resulted in  lack of effective communication between commissioners and performance issues being missed.

  • Informed by a survey of over 80 senior local government leaders, this report finds public health teams successfully working alongside their new local government colleagues to focus not just on the traditional big public health issues such as sexual health, obesity, smoking cessation but to explore new and innovative ways to tackle the wider social, cultural and environmental determinants of health. 
  • Recommendations include a review of data sharing and a review of the operation of the new health system in 2015 to ensure that Health & Wellbeing Boards have real influence.

Objectives of the NHS Commissioning Board

  • According to a letter from the Health Secretary to the NHS Commissioning Board Authority the Department of Health will hold the Authority to account for its performance against four strategic objectives, relating to:
    • transferring power to local organisations
    • establishing the commissioning landscape
    • developing specific commissioning and financial management capabilities
    • developing excellent relationships.
  • According to the HSJ the letter follows concern the NHSCB may attempt to retain more control over CCGs than intended by the health secretary.
