  • The BHF House of Care programme aimed to encourage the adoption of care and support planning using the House of Care Framework for people with heart and circulatory LTCs. 
  • Findings of the three year programme (2015-18) show that 41 general practices across five sites have successfully introduced a care and support planning approach as part of our programme. All sites have made sustainable shifts towards person-centred care.

Coalition for Collaborative Care and their House of Care

The Coalition for Collaborative Care
  • "The Coalition for Collaborative Care is a group of individuals and organisations across the health, social care and voluntary sectors that want to make person-centred, coordinated care a reality for people living with long-term conditions." 
  • The model under development by the Coalition is the "House of Care" which was developed and tested by the Year of Care programme and People Powered Health. It is a flexible blueprint that uses collaborative care and support planning and at the same time recognises the importance of non-medical, community-based activities and resources. 
  • The website describes the House of Care model, includes a blog and seeks to support local community efforts to build their own House of Care.

House of Care - patient centred care at all levels

  • “The House of Care will enable individuals to make informed decisions which are right for them, and empower them to self-care for their long term conditions in partnership with health and care professionals.”
  • The House relies on four key interdependent components -commissioning for system improvement;
  • engaged, informed individuals and carers; processes structured around the needs of patients and carers using the best evidence available and health and care professionals working in partnership.
