Testing the RightCare model

  • The RightCare model of identifying opportunities for savings and quality improvements benchmarks CCGs against the "Best 5" of "Similar 10" CCGs. The author tests this model using public data on cancer mortality and concludes that the ‘Similar 10’ are inappropriate comparators and CCGs cannot expect to match the average performance of the RightCare ‘Best 5’.

4: Chief Executive's report

  • Simon Stephens reinforced the following points:
    • It not permissible for a GP practice to charge NHS patients for appointments.
    • CCGs cannot change the national contrac relating to QOF.
  • The Five year Forward View delivery plan for 2018/19 - 2019/20 will be published at the end of March. It is not a bidding paper but an examination of realistic deliverables to 2020.
  • "STPs are here to stay" The delivery plan will include 
    • the formal appointment process for STP leads
    • STPs will take resources from across the system to beef up its process
    • decision rights to facilitate reconfiguration changes to statutory organisations to support its work

  • Data for each CCG is compared to the 10 most demographically similar CCGs. 
  • Headline opportunity areas are: potential lives saved per year, bed days, elective and non-elective spend, primary care prescribing, and includes opportunities for quality improvement and spend differences for a range of programme areas.
  • The report includes a detailed look at 19 'Pathways on a page' with difference from the average of the 10 most similar CCGs.

  • NHS Right Care has compiled data packs of Commissioning for Value packs published in October 2016, but collated at an STP footprint level. The information in these packs include headline opportunities, improvement opportunity tables and pathways on a page showing how CCGs in each STP differ from their peers.

  • Commissioning for Value Focus packs provide detailed information at CCG level on the highest spending programme areas. 
  • Areas now include 
    • cancer (May 2016) 
    • the CVD family of conditions including renal, stroke and diabetes (Apr 2016) 
    • maternity and early years (April 2016) 
    • mental health and dementia (May 2016) 
    • musculoskeletal and trauma (May 2016) 
    • neurology (April 2016) 
    • respiratory. (April 2016) 
  • The packs include a wider range of outcome measures and information on the most common procedures and diagnoses for the condition in question.

  • ‘Where to Look’ packs refreshed for 2016 - a data pack for each CCG as pdf and the accompanying data and methodology spreadsheets. 
  • Benchmarking data including
    • 10 CCGs who have the most similar population.
    • potential improvements around admissions, prescribing, potential lives saved
  • Data refresh from 14 'Pathway on a Page' packs 
  • Complex patients - Analysis of complex patients (as defined by inpatient admissions, outpatient and A&E attendances) for the 2% of patients that the CCG spends the most on for inpatient admissions in 2013/14. Includes analysis by age, spend by morbidity and A&E.
  • Publication designed to coincide with NHS Shared Planning Guidance 2016/17 - 2020/21.
