- The paper recommends that a 4-tier clinical service be developed: a) Tier 1: resources and support for self-care. b) Tier 2: generalist care including a therapeutic relationship in general practice and a community-based interdisciplinary rehabilitation service led by allied health professionals. c) Tier 3: specialist care including system-based investigation, management and rehabilitation. d) Tier 4: specialist management of specific complications.
