- GP at Hand is a primary care practice that incorporated a ‘digital-first’ service model into an existing practice in 2017. The practice operates in North West London, commissioned through a GMS contract through NHS Hammersmith and Fulham CCG. The practice now offers a ‘digital-first’ model of primary care, primarily through use of a mobile app and video consultations provided by their subcontractor, Babylon.
