Using risk stratification in new care models to identify vulnerable patients

  • Analysts at NHS England have looked at the approaches taken by five new care model vanguards (Tower Hamlets, Sunderland, North East Hants and Farnham, Erewash and Morecambe Bay) to identify patients at risk in their communities. They found that they have introduced the ‘building blocks’ of an effective risk stratification process and most are using predictive models and implementation is clinically-led.
