NHS finances - incentives improve process but do not impact on patient outcomes

  • This research report reviews different approaches to payment for health services in the English NHS and explores the evidence on whether recent payment initiatives have met their goals. 
  • Approaches examined are: Primary care Quality and Outcomes Framework; Activity-based payment in secondary care; Pay-for-performance schemes in secondary care and Block contracts in secondary care. 
  • While the evidence supports a role for financial incentives in improving the quality and productivity of processes of care there is a lack of evidence for an impact on patient outcomes.
