- A review of the quality of care and treatment provided by 14 NHS trusts and NHS foundation trusts which were persistent outliers on mortality indicators.
- The review process was designed by a team of clinicians and other key stakeholders, based on the NHS National Quality Board guidance and was guided by the following principles:
- Patient and public participation
- Listening to the views of staff
- Openness and transparency
- Co-operation between organisations
- The review identified some common challenges facing the wider NHS resulting in a number of ambitions for improvement within two years. Key outcomes for commissioners include:
- Ambition 1 - development of a new national indicator on avoidable deaths in hospitals, measured through the introduction of systematic and externally audited case note reviews.
- Ambition 2 - importance of rapid access to accurate, insightful and easy to use data about quality at service line level.
- Ambition 3 - involvement of patients, carers and members of the public as vital and equal partners partners in the design and assessment of their local NHS
- Ambition 4- Compilation of data and soft intelligence through the recently formed network of Quality Surveillance Groups
- Ambition 5 - Highlight the importance of research & innovation, and involvement of Academic Health Science Networks
- Ambition 6 - publication of evidence-based staffing levels and publication of a ‘How to’ guide on getting staffing right for nursing form the National Quality Board
- Ambition 7 - support for training junior doctors as clinical leaders
- Ambition 8 - developing innovative ways of engaging staff
