Learning from deaths: Guidance for NHS trusts on working with bereaved families and carers

  • This guidance is for NHS trusts, NHS foundation trusts and for services provided through NHS England specialised commissioning. It consolidates existing guidance and provides perspectives from family members who have experienced a bereavement within the NHS. It details how trusts should support and engage families after a loved one’s death in their organisation’s care.
  • This guidance is complemented by Information for families following a bereavement (Annex 1) 
  • Responsibilities of CCGs
    • "CCGs should should familiarise themselves with this guidance, as they are responsible for commissioning services from trusts and have a monitoring and assurance role." p9
    • "Where the deceased person received care in a community setting, or from different trusts, normally the lead care provider will be families’ key point of contact. It should offer bereavement support, and expect co-operation and collaboration from all the providers involved. Where there is disagreement about which organisation should take the lead in supporting the family, CCGs should take this decision in consultation with the providers involved." p18
    • Complaints can be made to the relevant CCG instead of NHS trust directly involved. p41
