• A description of a real time approach taken in Ealing to demonstrate the impact of services on patient movement across primary care /acute care service boundaries, bringing together SUS and Rio data. The approach was tested on the impact of case management of patients with LTCs on their hospital admissions and to demonstrate the impact of approaches taken by different GP practices on hospital bed days.
