- This report is based on the discussions at a meeting involving representatives of eight vanguard sites including:
- a strategic partnership with a combined budget for health and social care
- four multispecialty community providers
- a single local GP partnership
- a GP super-practice
- an acute care collaboration
- The report describes novel care pathways, including physical and mental health, cancer, alcohol misuse and finances, and concludes that the NHS must invest in new technology if innovative models of care being trialled by its vanguards are to be rolled out.
