- This letter confirms the arrangements for the selection of eligible practices, the nature of the support offer and how this will be secured, funding arrangements and monitoring and evaluation.
- Local NHS England teams will need to be able to confirm by 28 January 2016 details of those GP practices they have agreed to support.
